Colorado's Center for Improving Value in Healthcare (CIVHC) examines the distribution of certain chronic conditions across the state. This publication assesses these chronic conditions in terms of geography, payer type, gender, and age. Trends in disease prevalence across time are also discussed. CIVHC accessed 2012-2015 data from the Colorado All-Payer Claims Database (APCD) for this report.
